## 1. Purpose

One table of everything the department inspects, tests, services, reviews or renews on a cycle: the interval, who does it, who is qualified to do it, the standard it is done against, the record it leaves and the month it falls in. The schedule is the backbone of the Monitoring category of the audit and the source of half the rows in the Corrective Action Register. If an item is not on this table, nobody owns its cycle.

The intervals are the package’s own, set at or inside what the standards and the applicable provincial or territorial regulations require. Confirm each standard’s clause against the current edition; the interval applies either way. Each program module carries the detailed checksheet for its rows; this note is the master calendar. The applicable OHS regulation sets how often tools and equipment must be inspected and by whom, and how long the records are kept ([J15 Equipment Inspection, Ladders and Aerial Devices](https://thehumanfactor.ca/library/fire-service-safety/j15-equipment-inspection-ladders-and-aerial-devices)). This schedule has equipment inspected by its user before each use and by a competent person at least annually, keeps the records five years ([05 Document Control and Records](https://thehumanfactor.ca/library/fire-service-safety/document-control-and-records)), and sets several rows more often than that because the standard or the hazard demands it.

> **Note: Customize**
>
> Replace the month column with the department’s own months once the fiscal year and the fit-test and medical contracts are set. Keep every row. A row the department does not do yet stays on the table with “not yet scheduled” in the month column, so the gap is visible to the audit and the management review.

## 2. How to read the table

- **Interval**: how often. “After use” means after every use or exposure. “Annual” means within 12 months of the last.
- **Done by**: the role that performs it. **Qualified**: the competence required, so that “done by the crew” never means “done by whoever was free”.
- **Standard**: the edition and clause.
- **Record**: where the evidence lives ({{Records system}} unless stated).
- **Month**: default month for annual and less frequent items in an April to March fiscal year; shift with [04 Annual Program Cycle and Calendar](https://thehumanfactor.ca/library/fire-service-safety/annual-program-cycle-and-calendar).
- **Stage**: the stage at which the row becomes required.

|:---|:---|:---|
| Owner | Senior officer responsible for H&S | Accountable that the schedule is complete, current and resourced |
| Doer | The “done by” role in each row; program managers for their rows; fleet and facilities under their service-level agreements | The table |
| Verifier | HSO samples completion monthly (indicator: scheduled items completed on time, target 95 per cent); internal audit category 18 samples records yearly; JOHSC reviews the completion rate quarterly | [Checklist - Monitoring, Inspection and Indicators](https://thehumanfactor.ca/library/fire-service-safety/checklist-monitoring-inspection-and-indicators) |
| Feedback | Completion rate in the monthly indicator report; missed items listed at the operations meeting; program teams see their rows in the quarterly update | [03 Accountability and Reporting](https://thehumanfactor.ca/library/fire-service-safety/accountability-and-reporting) |
| Closure | Every missed or overdue item and every deficiency found enters the [Corrective Action Register](https://thehumanfactor.ca/library/fire-service-safety/corrective-action-register); equipment that misses its test is tagged out until tested | Register rules |

**Loop test.** The flow test contractor cannot reach a volunteer station in July. The HSO sees the gap in the August completion report, the units are swapped with tested reserve units within the week, the station’s test is rebooked for September, the register carries the item, and the station officer tells the members why the SCBA on the truck changed.
