**Purpose.** The department’s record of who is due for what and whether it was done, holding dates, tiers, status flags and the fitness-for-duty category only. It contains no results, diagnoses, laboratory values, audiogram thresholds or fitness scores; those stay with the custodian (the fire department physician, clinic or peer fitness trainer) under the information-sharing protocol in [Medical, Fitness and Wellness Program](https://thehumanfactor.ca/library/fire-service-safety/medical-fitness-and-wellness-program) section 17.9. It is the evidence for the medical, hearing and respiratory programs’ completion indicators and for the audit [(NFPA 1580, 2025)](https://thehumanfactor.ca/library/fire-service-safety/form-annual-medical-and-fitness-completion-tracker#ref-nfpa-1580); (CSA Z94.4-18). One row per member; updated as determinations arrive. (S1)

## Fields

| Field | Type | Required | Who completes |
|:---|:---|:---|:---|
| Roster and tier (Part A) | ID, choice | Yes, every member | Program manager from HR and roster |
| Medical dates and category (Part B) | Dates, choice, task numbers | Yes | HSO from the physician’s determination |
| Cohort flags | Y/N | Yes | Physician (flag only) |
| Respirator, fit test, hearing, vision (Part C) | Dates, choice, Y/N | Yes for SCBA-qualified and operators | HSO; Respiratory Protection program administrator |
| Fitness and immunization (Part D) | Dates, choice | Yes | Peer fitness trainer (completion only); clinic (status flags) |
| Quarterly summary (Part E) | Percentages, counts | Quarterly | Program manager |

## Instructions

1. Build the tracker in the records system with row-level access limited to the program manager, the HSO and the Respiratory Protection program administrator; station officers see only their members’ due dates and “current / overdue”.
2. Enter only what the custodian sends: a category, a flag, a date. If a result arrives, return it to the custodian and log the privacy event.
3. Run the overdue list monthly; an overdue SCBA clearance restricts the member from SCBA assignment in the roster until cleared.
4. Produce Part E quarterly for the program update and the JOHSC. Report volunteers by tier so a gap cannot hide in the department average.
5. Retain for the duration of service plus 30 years with the exposure records (OSHA, n.d.-a, 29 CFR 1910.1020, as the benchmark).

> **Tip: Volunteer and composite departments**
>
> Schedule clinic nights from the overdue list: the tracker tells you how many Tier 1 and Tier 3 evaluations a station night must cover.
