**Purpose.** The officer’s record, per incident, that preliminary exposure reduction happened at the scene and post-call decontamination happened at the station for every exposed member, and that the apparatus, tools and SCBA were cleaned before the unit returned to service [(NFPA 1585, 2025, ch. 7 to 9)](https://thehumanfactor.ca/library/fire-service-safety/form-decontamination-log#ref-nfpa-1585); [(NFPA 1550, 2024, ch. 16)](https://thehumanfactor.ca/library/fire-service-safety/form-decontamination-log#ref-nfpa-1550). It is cross-referenced to each member’s [Form - Exposure Report](https://thehumanfactor.ca/library/fire-service-safety/form-exposure-report) by incident number. One log per unit per incident. (S1)

## Instructions

1. Start Part A at the scene; the on-scene SWP’s checklist card feeds it. Finish Parts B to D before the end of the shift, or within 24 hours for a volunteer unit.
2. A “N” in any Part B column is not a fault; it is information. Write why in the notes, and raise it at the hot debrief. Learning information from this log is never used for discipline.
3. The exposure report column is reconciled monthly by the {{Health and Safety Officer}}: every “Y” in Part B should have a report with the same incident number.
4. Send the log to the {{Decon Lead}} weekly. Logs are kept for the retention period in [05 Document Control and Records](https://thehumanfactor.ca/library/fire-service-safety/document-control-and-records) and are summarized, not copied, in the quarterly update.

> **Tip: Volunteer and composite departments**
>
> For a crew that goes home from the scene, Part B’s shower column records “home within 60 min” and Part C’s bagged-gear row records the tote left at the hall for pickup.
