## Purpose and use

One record per driver, opened when a member starts the driver program and kept current for the member’s service. The training division keeps it; company officers check the authorization in Part C before assigning a driver; the licence and abstract are rechecked every year in Part B; every driver is re-evaluated at least every 24 months and after any collision or at-fault near miss in Part F ([Policy - Apparatus and Driving](https://thehumanfactor.ca/library/fire-service-safety/policy-apparatus-and-driving) 5.5 and 5.6; [Apparatus, Driving and Roadway Operations Program](https://thehumanfactor.ca/library/fire-service-safety/apparatus-driving-and-roadway-operations-program) sections 5.5, 5.6 and toolkit T9). Operator content follows the driver/operator job performance requirements [(NFPA 1010, 2024, ch. 11)](https://thehumanfactor.ca/library/fire-service-safety/form-driver-qualification-and-training-record#ref-nfpa-1010) and the health and safety standard’s driver rules [(NFPA 1550, 2024, ch. 8)](https://thehumanfactor.ca/library/fire-service-safety/form-driver-qualification-and-training-record#ref-nfpa-1550); training records are managed to the training standard (CSA Z1001-18). The record holds the medical outcome only, never a diagnosis.

## Part A: Member and licence

| Field | Entry |
|:---|:---|
| Member identifier, station, service status (career / volunteer / paid-on-call) | |
| Licence class and endorsements (for example air brake), against the class your jurisdiction requires for each apparatus type ([J30 Emergency Vehicles, Drivers and Licensing](https://thehumanfactor.ca/library/fire-service-safety/j30-emergency-vehicles-drivers-and-licensing)) | |
| Licence expiry date and any conditions | |
| Driving abstract: date obtained and result (no concerns / concerns referred to the program manager) | |
| Medical fitness to drive from {{Occupational Health Provider}}: fit / fit with restriction / not fit; restriction in task terms; date | |

## Part B: Annual licence and abstract checks

| Year | Licence status checked (date) | Abstract checked (date) | Result | Checked by (role) |
|:---|:---|:---|:---|:---|
| | | | | |

## Part C: Driver program and authorization by apparatus type

| Apparatus type (light, pumper, aerial, tender, rescue, wildland, tiller) | Classroom (date) | Closed course (date) | Supervised hours, at least {{Hours}}, including night and adverse weather | Road evaluation (date; evaluator role) | Operator qualification or certification (pump, aerial, tender) | Tender rollover awareness (tenders only) | Authorized (date; by role) |
|:---|:---|:---|:---|:---|:---|:---|:---|
| | | | | | | | |

## Part D: Supervised driving log

| Date | Apparatus | Hours | Conditions (day / night / rain / snow / ice) | Supervising officer (role) and initials |
|:---|:---|:---|:---|:---|
| | | | | |

## Part E: Other required training

| Item | Date completed | Next due |
|:---|:---|:---|
| Traffic incident management, responder level [(NFPA 1091, 2024)](https://thehumanfactor.ca/library/fire-service-safety/form-driver-qualification-and-training-record#ref-nfpa-1091) | | Three years |
| Backing and spotter signals; seat belt and occupant rules | | Annual drill |
| Response mode matrix and intersection procedure | | With re-evaluation |
| Telematics briefing: what is collected, who sees it, how it is used | | On change |
| Personal vehicle response acknowledgement (members who respond from home or work) | | Annual |

## Part F: Re-evaluations

| Date | Reason (24-month cycle / after collision / after at-fault near miss / return from absence / licence change) | Result (pass / remediation) | Evaluator (role) | Next due |
|:---|:---|:---|:---|:---|
| | | | | |

## Part G: Removal from driving and reinstatement

| Date | Reason (licence suspended, downgraded or conditional / medical restriction / re-evaluation not yet met / member’s own request) | Apparatus types removed | Reinstated (date; by role; evidence) |
|:---|:---|:---|:---|
| | | | |

Removal is a fitness and qualification matter, recorded here and not in a discipline file. A member who says they are not fit to drive is removed for that duty period without consequence and is not recorded in Part G.
