## How to use

Completed during the management review by the senior officer responsible for H&S, read back at the close, signed by the fire chief, and issued within 10 working days to attendees, program managers and the JOHSC; a member summary (Part G) reaches every station within 30 days ([Management Review](https://thehumanfactor.ca/library/fire-service-safety/management-review) sections 2 and 6). Sections A to E record the outputs ISO 45001 requires of a management review: conclusions on suitability, adequacy and effectiveness; improvement opportunities; changes to the system; resources; actions; and communication to workers (ISO 45001:2018, cl. 9.3). Every action and objective receives a [Corrective Action Register](https://thehumanfactor.ca/library/fire-service-safety/corrective-action-register) ID so that next year’s first agenda item can be answered from the register. The record is retained for 10 years.

## Part 0: Header

| Field | Entry |
|:---|:---|
| Review date and location | |
| Fiscal year reviewed | |
| Chair | Fire chief |
| Attendance (roles) | Required: deputy chiefs; senior officer H&S; HSO; division chief, training; JOHSC worker co-chair; JOHSC employer co-chair; program managers presenting; corporate safety or HR representative; finance liaison. Observers: union; volunteer association |
| Pack issued on (date; 10 working days before) | |
| Inputs received (tick each): last year’s decisions; indicator report; audit results; register status; JOHSC recommendations and responses; program appraisals; incident and learning summary; culture and psychological-safety results; legal and standards changes; resource needs; proposed objectives | |
| Inputs missing and why | |

## Part A: Statement on suitability, adequacy and effectiveness

| Question | Conclusion (with the evidence relied on) |
|:---|:---|
| Is the safety management system suitable for the services the department provides and the hazards in its profile? | |
| Is it adequately resourced (people, time, money, records system)? | |
| Is it effective on the evidence (program score, effectiveness index, maturity band, indicator trends, culture results, psychological-safety measure)? | |
| Exceptions: where the answer is no, and what Part E does about it | |
| Policy review: OHS policy reviewed in consultation with the JOHSC; reaffirmed or amended (version) | |

## Part B: Improvement opportunities

| Source (audit OFI, appraisal, learning review, JOHSC, peer practice) | Opportunity | Decision (adopted / declined / deferred) | Reason | If adopted: owner (role), due, register ID |
|:---|:---|:---|:---|:---|
| | | | | |

## Part C: Changes to the system

| Change (policy, program, procedure, module switched on or off, governance, team charter, RACI, records system, integration with fleet, facilities, HR, procurement, finance) | Reason | Owner (role) | Due | Consultation required (JOHSC, union, association, counsel) | Register ID |
|:---|:---|:---|:---|:---|:---|
| | | | | | |

## Part D: Resources

| Item (from the consolidated budget worksheet or the appraisals) | Safety-critical? (legal duty or critical control) | Decision (fund now / submit in budget / defer / decline) | Amount and year | Reason | Owner (role) |
|:---|:---|:---|:---|:---|:---|
| | | | | | |

Safety-critical items not funded, stated plainly for the annual report to council (the formal notice of gaps):

## Part E: Actions

| \# | Action | Owner (role) | Due | Priority (P1 to P4) | Register ID | Reported to |
|:----|:-------|:-------------|:----|:--------------------|:------------|:------------|
| | | | | | | |

Priority is the department’s one scale, the same one the [Corrective Action Register](https://thehumanfactor.ca/library/fire-service-safety/corrective-action-register) uses for every deficiency; the due date is the one the priority sets unless the fire chief sets an earlier one. P1 (Imminent): interim control immediately, the same shift, with the fire chief and JOHSC co-chairs told within 24 hours; plan due in two working days; closed within seven days. P2 (Serious): interim control within 24 hours where practicable; plan due in seven days; closed within 30 days. P3 (Moderate): plan due in 30 days; closed within 90 days. P4 (Improvement): plan by the next quarterly program update; closed within 365 days or the next budget cycle. Extensions and escalation follow the register.

|:---|:---|:---|:---|:---|:---|:---|:---|:---|
| | | | | | | | | |

## Part G: Communication plan and member summary
