## Purpose and use

For every collision, backing contact, rollover, roadway struck-by and near miss involving a department vehicle, or a member’s own vehicle on response ([Policy - Apparatus and Driving](https://thehumanfactor.ca/library/fire-service-safety/policy-apparatus-and-driving) 5.15). The driver and officer complete Parts A to D before the end of the duty period; the health and safety officer completes Part E. Any injury is also reported on [Form - Injury and Incident Report](https://thehumanfactor.ca/library/fire-service-safety/form-injury-and-incident-report). The fields follow the program’s collision report summary ([Apparatus, Driving and Roadway Operations Program](https://thehumanfactor.ca/library/fire-service-safety/apparatus-driving-and-roadway-operations-program) toolkit T7) and the incident investigation standard (CSA Z1005:21); the health and safety standard expects every vehicle collision and near miss to be investigated [(NFPA 1550, 2024, ch. 6)](https://thehumanfactor.ca/library/fire-service-safety/form-vehicle-collision-report#ref-nfpa-1550).

> **This form records facts** for reporting, insurance and triage. Describe what happened in plain words; do not assign fault. The learning review, if the event is triaged for one, is held separately and is never used for discipline [(The Human Factor, 2026a)](https://thehumanfactor.ca/library/fire-service-safety/form-vehicle-collision-report#ref-the-human-factor-2026a).

## Jurisdiction requirements

This chapter relies on the following legal requirements, which differ by province and territory. Check your jurisdiction’s section on each topic page before adopting it.

| Topic | What to check in your jurisdiction | Page |
|:---|:---|:---|
| Incident Reporting and Investigation | Which injuries and occurrences must be reported to the regulator and how fast, who else must be told (such as the committee), the employer’s deadline to report to the compensation board, and whether that report also counts as notice to the regulator (Part E) | [J08 Incident Reporting and Investigation](https://thehumanfactor.ca/library/fire-service-safety/j08-incident-reporting-and-investigation) |
| Emergency Vehicles, Drivers and Licensing | When the traffic statute requires a collision to be reported to police (an injury, or damage above a prescribed amount) and any other duties of an emergency vehicle driver after a collision (Part E) | [J30 Emergency Vehicles, Drivers and Licensing](https://thehumanfactor.ca/library/fire-service-safety/j30-emergency-vehicles-drivers-and-licensing) |
