The courses

Course details

The problem: a service cannot learn from events until everyone means the same thing by learning. What we do: six lessons of about ten minutes each: why learn from events; hindsight and the "human error" label; how events happen in systems; just culture and the learning firewall; the three tiers and Event Triage; caring for people and closing the loop. What you get: the shared language for everything that follows, and the prework for every other course. How long: about an hour, self-paced, free and open to anyone.

Suggested pathways

  • JHSC members and health and safety representatives: ATC 0, then ATC 1.
  • Safety officers, supervisors, and patient safety or quality leads: ATC 0, then ATC 2. Consider ATC 1 alongside your JHSC so you share a method.
  • Senior leaders: ATC 0, then ATC 3.
  • A service adopting the manual: leaders through ATC 3, the JHSC through ATC 1, and the people who will facilitate Tier 1 and Tier 2 reviews through ATC 2.

Open-enrolment cohorts run with 10 to 20 participants (ATC 3 up to 30). Joint labour and management cohorts are encouraged.

Dates and cohorts

Ask about dates or an in-house cohort

Tell us which course, roughly how many people, and whether you would like open seats or a private cohort for your service. We will come back with dates.
