Form - PPE Cleaning and Repair Log

Document FSSMS-FRM-18Stage 1Revision 5Version 0.3.3

Topic pages open on your jurisdiction, and requirements tables show its line. You can change this on any page.

Purpose. The record, per element and per cycle, of every advanced and specialized cleaning, every routine cleaning that followed a fire exposure, and every repair, with the extractor’s calibration and maintenance block. It is the evidence that cleaning happened as the standard requires and that the machine was within its limits (NFPA 1850, 2026, ch. 6 and 8). Without this log, “advanced cleaning” is a claim, not a record. (S1)

Instructions

  1. One line per element per cycle. A load of six coats is six lines with the same extractor program entry.
  2. Confirm the extractor’s program, G-force and water temperature are within the standard’s and the manufacturer’s limits before every load; if either is unknown, do not run the load.
  3. Specialized cleaning is never started until Part D is completed. Gear with an unknown or unremovable contaminant goes to a retirement record.
  4. No repair without an entry in Part B and no return to service without the post-repair advanced inspection reference.
  5. The {{PPE Coordinator}} reconciles Part A against working fire incident reports monthly: every exposed ensemble should have an after-exposure entry within seven days.
  6. Keep the log for the life of the elements on it plus the retention period in 05 Document Control and Records; keep ISP batch records and chain-of-custody sheets with it.
