Form - Personnel Accountability Report Log

Document FSSMS-FRM-28Stage 1Revision 4Version 0.3.3

Topic pages open on your jurisdiction, and requirements tables show its line. You can change this on any page.

Purpose and use

The paper record behind the accountability board. Part A is kept at each entry control point by the entry control officer; Part B is kept by the accountability officer or command aide at the command post; Parts C and D close the incident out. It is used at every incident where a member enters an IDLH or hazard zone, and it is the record that proves the department could say, at any moment, who was inside, where, since when and with how much air (NFPA 1550, 2024, s. 21.5). The entry control fields follow the elements that fire service entry control guidance names: identity, task, location, time of entry and air remaining (MLITSD, n.d., GN 5-1). It answers the accreditation criterion for accountability of all personnel at all risk events (CPSE, 2019, criterion 11A.12).

Printed on waterproof stock, two copies per apparatus and six per command vehicle, with a grease pencil. Where the department uses an electronic system, this form is the fallback and is carried regardless.
