Purpose and use

Part A is completed by the lead instructor before every practical evolution and reviewed with the crew at the start; a recurring drill may use a standing assessment with a “today” check. Part B is added for every high-hazard evolution. Parts C to E are added for live fire: the pre-burn plan before the day, the go/no-go card before ignition, and the record after the burn (SWP - Live Fire Training Evolutions; Training Safety and Live Fire Program sections 5.1, 5.2, 5.5 and toolkit T1 and T2). The live fire content follows the live fire standard (NFPA 1403, 2024), now consolidating into the training standard (NFPA 1400, 2026), and the training chapter of the health and safety standard (NFPA 1550, 2024, ch. 7).

Part A: Evolution risk assessment (every practical evolution)

Field Entry
Evolution, date, place; objective and the standard it trains to; standing assessment reference and today check (if recurring)
Lead instructor (role); participants (number); prerequisites checked (yes / no)
Hazards (tick): thermal / fall / struck-by / cut / cardiac and heat strain / cold / water / electrical / vehicle / respiratory / structural / noise / other
Controls in order: elimination; substitution; engineering; administrative; PPE (the same PPE and accountability as operations: yes / exceptions)
Staffing and ratios; safety role named (lead instructor or training safety officer)
Weather limits for today and the reading at the start (humidex or heat index, wind chill, wind, lightning); rehab and hydration: lead (role), water, shade or warmth, log
Emergency plan: who calls; ambulance location or response time; removal route; hospital; stop triggers
Debrief question; signatures of the lead instructor and, where required, the training safety officer

Part B: High-hazard additions

Field Entry
Type: live fire / acquired structure / roof / rope and heights / water and ice / confined space or trench / driver course / power tools / flashover simulator / more than 15 participants / other named
Training safety officer (role), confirmed with no other duty and briefed on the stop authority
Structure or prop: annual inspection in date and today’s pre-use check done (SWP - Training Tower and Prop Inspection)
Rescue plan for the hazard (rapid intervention crew, rope or water rescue, confined space retrieval)
Medical status for exertion checked for every participant and instructor (yes / no); EMS on site or response time

Part C: Live fire pre-burn plan (complete before the day; tick when done)

# Item Done
C1 Site plan: fire room, egress routes, apparatus, water supply, rapid intervention crew and backup line positions
C2 Structure or prop inspected and signed; for an acquired structure the acquisition checklist is complete (owner permission, asbestos clearance, utilities disconnected in writing)
C3 Fuel: Class A listed materials only; type, quantity and location for each evolution
C4 Roles by position: instructor-in-charge, safety officer, ignition officer, an instructor per crew, rapid intervention crew, backup line, pump operator, EMS, rehab, entry control
C5 Ratio no more than five participants per instructor on interior evolutions; maximum interior evolutions per instructor per day
C6 Water supply primary and backup, with flows; communications plan and channel; evacuation signal; accountability method identical to operations
C7 EMS and rehab arrangements; hospital route; weather limits
C8 Participant prerequisites and medical status on the roster; instructor qualifications on the register; permits and notifications; safety officer’s review and signature

Part D: Go/no-go card (read aloud; every line yes or the burn does not proceed)

# Precondition Yes / No
D1 Safety officer present, independent, no other duty
D2 Every participant and instructor walked through; evacuation signal demonstrated
D3 Fire room, fuel load and location match the plan; nothing prohibited present
D4 One fire only; none in an egress route; none below participants
D5 Rapid intervention crew on air and in position; backup line charged, flowed and staffed by an instructor
D6 Water supply confirmed; pump operator in place
D7 Radio check complete; accountability tags in; personnel accountability report complete
D8 Ratio counted; EMS on site; rehab ready; weather inside limits
D9 Every participant’s PPE and SCBA in service condition; nobody in retired gear
D10 Ignition officer named; nobody else lights a fire
Instructor-in-charge and safety officer signatures; time of go

Part E: Record after the burn

Field Entry
Evolutions run, times and fires lit per evolution; weather readings through the day; structure or prop reinspected after each evolution (yes / no; findings)
Rehab checks between evolutions (count); anyone held out on medical grounds (count)
Injuries, exposures and near misses (counts; report references); low-air alarms inside (count; near-miss reports)
Stop calls (count; by role; reason in brief); self-removals (count, recorded only as “self-removed”)
Debrief held with everyone (yes / no); answers to the debrief question in brief; exposure reports filed for all
Filed in {{Records system}} (date; by role)

Fields

Field Type Required Who completes
Part A fields Text; ticks; numbers; signatures Yes for every practical evolution Lead instructor; training safety officer where required
Part B fields Pick list; text; yes/no Yes for every high-hazard evolution Lead instructor; training safety officer
Part C items Tick and date Yes for every live fire evolution, before the day Instructor-in-charge; safety officer signs C8
Part D lines Yes / No; signatures; time Yes, before ignition Instructor-in-charge and safety officer together
Part E fields, roster and logs Numbers; text; yes/no; attachments Yes after every live fire day Instructor-in-charge

Retention. Live fire records seven years by default; other evolutions at least five years (05 Document Control and Records). Medical status is recorded as yes or no only.

Jurisdiction requirements

This chapter relies on the following legal requirements, which differ by province and territory. Check your jurisdiction’s section on each topic page before adopting it.

Topic What to check in your jurisdiction Page
Fire Service OHS Rules and Fireground Operations Any statutory buddy, two-in/two-out or rapid intervention rule, and whether it applies in training as it does on the fireground (Parts C and D) J12 Fire Service OHS Rules and Fireground Operations
Firefighter PPE and Protective Equipment The protective ensemble, SCBA and PASS standards a regulation names for structural firefighting, which apply in training as well (line D9) J13 Firefighter PPE and Protective Equipment
Respiratory Protection, Fit Testing and Breathing Air How often SCBA users must be fit tested and whether the test must be quantitative, which applies to every live fire participant (line C8) J14 Respiratory Protection, Fit Testing and Breathing Air
WHMIS, Hazardous Materials and Asbestos The asbestos survey and abatement rules that must be met before any work in an acquired structure (line C2) J21 WHMIS, Hazardous Materials and Asbestos
Wildland and Interface Fire Burn permits and burn restrictions set by the wildland fire agency, alongside any municipal bylaw (lines C2 and C8) J35 Wildland and Interface Fire
