Form - Apparatus Daily and Weekly Check Sheet

Document FSSMS-FRM-31Stage 1Revision 4Version 0.3.3

Topic pages open on your jurisdiction, and requirements tables show its line. You can change this on any page.

Purpose and use

One sheet per unit per week, kept in the cab or in {{Records system}}. Career stations complete Part A at the start of every duty period; volunteer stations complete it at least weekly and after every use. Part B is done once a week. Mark each line ✓ (pass), X (defect) or N/A. Any X is written up in Part D the same day, and a unit that meets any line in Part C does not respond until a technician returns it. The station officer reviews the month’s sheets in Part E. The content follows the program’s check list and out-of-service criteria (Apparatus, Driving and Roadway Operations Program sections 5.2 and toolkit T1 and T2) and the in-service vehicle standard (NFPA 1910, 2024); the health and safety standard expects apparatus to be inspected and kept in service condition (NFPA 1550, 2024, ch. 8).
