Form - Vehicle Collision Report

Document FSSMS-FRM-33Stage 1Revision 4Version 0.3.3

Topic pages open on your jurisdiction, and requirements tables show its line. You can change this on any page.

Purpose and use

For every collision, backing contact, rollover, roadway struck-by and near miss involving a department vehicle, or a member’s own vehicle on response (Policy - Apparatus and Driving 5.15). The driver and officer complete Parts A to D before the end of the duty period; the health and safety officer completes Part E. Any injury is also reported on Form - Injury and Incident Report. The fields follow the program’s collision report summary (Apparatus, Driving and Roadway Operations Program toolkit T7) and the incident investigation standard (CSA Z1005:21); the health and safety standard expects every vehicle collision and near miss to be investigated (NFPA 1550, 2024, ch. 6).

This form records facts for reporting, insurance and triage. Describe what happened in plain words; do not assign fault. The learning review, if the event is triaged for one, is held separately and is never used for discipline (The Human Factor, 2026a).
